Club Name:Wollumbin Dragon Boat Club (the “Club”)

Medical Treatment Consent, Waiver and Release of Liability

I wish to participate in dragon boating and associated activities with the Club.

Fitness and Medical Treatment Consent

I certify that I am medically and physically fit, and able to participate in dragon boating and associated activities. I say that I have received sufficient training for participation in the event and have not been advised otherwise by a qualified medical practitioner.

I hereby consent to receive medical treatment which may be deemed advisable in the event of injury, accident and/or illness during this event.

I acknowledge that this declaration will be used and relied upon by the Club, Queensland Dragon Boat Federation Inc., event holders, sponsors and organisers of events in which I may participate, as evidence of my fitness and ability to participate, and I agree to be bound by it.

Risks of this Sport

I acknowledge that:

1.dragon boating can be an inherently dangerous sport; and

2.the risks of serious injury, death or accident are not only inherent to paddlers, but are also present for volunteers and may arise from:

a. dangerous or defective equipment 

b. property owned, maintained or controlled by the Club, the Queensland Dragon Boat Federation Inc. or event organisers/holders

c. a person’s or group of people’s negligence or carelessness.

I have voluntarily read this warning and understood this warning and assume all of the risks inherent or otherwise of participating in dragon boating, associated activities of the Club and/or volunteering in this event.

I acknowledge that this Waiver and Release of Liability form will be used and relied upon by the Club, the Queensland Dragon Boat Federation Inc., event holders, sponsors and organisers of events in which I may participate and I agree to be bound by it.

In consideration of permitting me to participate in this event, I hereby take the following action for myself, my executors, administrators, heirs, next of kin, successors, and assign as follows:

A.Waivers, Release and Discharge from any and all liability form my death, disability, personal injury, property damage, property theft or actions of any kind which may hereafter accrue to me or my travelling to and from this event, THE FOLLOWING ENTITIES OR PERSONS: Queensland Dragon Boat Federation Inc. (QDBF), the Club, the QDBF and/or Club’s directors, officers, employees, volunteers, representatives or agents, the event holders, event sponsors, event directors and event volunteers;

BIndemnify and Hold Harmless the entities or persons mentioned in paragraph (A) from any and all liabilities or claims made by other individuals or entities as a result of any of my actions during this event.

This Waiver and Release of Liability shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law.

Other permissions:

I understand that at this event or related activities, I may be photographed. I agree to allow my photo, video or film likeness to be used for any legitimate purpose by the event holders, producers, sponsors, organisers and/or other representative assigns.
